Be it remembered, that on the 22nd day of September in the year of our lord one thousand eight hundred and eighty four personally appeared Santamaria Bartolomeo in the Court of Common Pleas, for the City and County of New York (said Court being a Court of Record, having common law jurisdiction, a Clerk and a Seal) and made his declaration to become a Citizen of the United States of America, in the words following, to wit:
I, Santamaria Bartolomeo do declare on oath, that it is bona fide my Intention to become a Citizen of the United States, and to renounce forever all allegiance and fidelity to any foreign Prince, Potentate, State or Sovereignty whatever, and particularly to the King of Italy of whom I am subject.
Sworn this 22 day of September 1884

An Attestation Whereof, and that the foregoing is a true copy of the original declaration of intention remaining of record in my office, I Nathaniel Jarvis Jr, Clerk of the said Court of Common Pleas, have hereunto subscribed my name and affixed the deal of the said Court, this 22 day of September 1884.
